消費生活用製品安全法の規定に基づく重大事故報告等に関する内閣府令 Cabinet Office Order on Report of Serious Incidents Based on the Provisions of the Consumer Product Safety Act 一部未施行 Partially unenforced 平成二十一年内閣府令第四十七号 Cabinet Office Order No. 47 of 2009 令和七年内閣府令第一二号 Cabinet Office Order No. 12 of 2025 消費生活用製品安全法の規定に基づく重大事故報告等に関する内閣府令（平成二十一年八月二十八日内閣府令第四十七号） Cabinet Office Order on Report of Serious Incidents Based on the Provisions of the Consumer Product Safety Act (Cabinet Office Order No. 47 of August 28, 2009) 消費生活用製品安全法（昭和四十八年法律第三十一号）第三十五条第二項及び消費生活用製品安全法施行令（昭和四十九年政令第四十八号）第六条第一号ロの規定に基づき、並びに消費生活用製品安全法を実施するため、消費生活用製品安全法の規定に基づく重大事故報告等に関する内閣府令を次のように定める。 Pursuant to the provisions of Article 35, paragraph (2) of the Consumer Product Safety Act (Act No. 31 of 1973) and Article 6, item (i), (b) of the Order for Enforcement of the Consumer Product Safety Act, and for the purpose of the enforcement of the Consumer Product Safety Act, the Cabinet Office Order on Report of Serious Incidents Based on the Provisions of the Consumer Product Safety Act is provided as follows. 第一条　この府令において使用する用語は、消費生活用製品安全法（以下「法」という。）及び消費生活用製品安全法施行令（以下「令」という。）において使用する用語の例による。 Article 1 The terms used in this Cabinet Office Order are the same as the terms used in the Consumer Product Safety Act (hereinafter referred to as the "Act") and the Order for Enforcement of the Consumer Product Safety Act (hereinafter referred to as the "Order"). （身体の障害） (Physical Disabilities) 第二条　令第六条第一号ロの内閣府令で定める身体の障害は、次に掲げるものとする。 Article 2 The physical disabilities specified by Cabinet Office Order referred to in Article 6, item (i), (b) of the Order are as follows: 一　次に掲げる視覚障害であって、長期にわたり身体に存するもの (i) the following visual impairments that are present in the body for a long period of time: イ　両眼の視力（万国式試視力表によって測ったものをいい、屈折異常がある者については、矯正視力について測ったものをいう。以下同じ。）がそれぞれ〇・一以下のもの (a) the visual acuities (meaning those measured using the international visual acuity test chart, or, for a person with an abnormality in refraction, those measured with corrected vision; the same applies hereinafter) in both eyes are 0.1 or less respectively; ロ　一眼の視力が〇・〇二以下、他眼の視力が〇・六以下のもの (b) the visual acuity in one eye is 0.02 or less, and the visual acuity in the other eye is 0.6 or less; ハ　両眼の視野がそれぞれ一〇度以内のもの (c) the visual fields in both eyes are 10 degrees or less respectively; or ニ　両眼による視野の二分の一以上が欠けているもの (d) one-half or more of the visual field using both eyes is lost; 二　次に掲げる聴覚又は平衡機能の障害であって、長期にわたり身体に存するもの (ii) the following impairments of hearing or equilibrium function that are present in the body for a long period of time: イ　両耳の聴力レベルがそれぞれ七〇デシベル以上のもの (a) the hearing levels of both ears are 70 decibels or more respectively; ロ　一耳の聴力レベルが九〇デシベル以上、他耳の聴力レベルが五〇デシベル以上のもの (b) the hearing level of one ear is 90 decibels or more, and the hearing level of the other ear is 50 decibels or more; ハ　両耳による普通話声の最良の語音明瞭度が五〇パーセント以下のもの (c) the best articulation score for ordinary speaking voices using both ears is 50% or less; or ニ　平衡機能の著しい障害 (d) serious impairment of equilibrium function; 三　次に掲げる嗅覚の障害 (iii) the following impairments of smell イ　嗅覚の喪失 (a) loss of smell; or ロ　嗅覚の著しい障害であって、長期にわたり身体に存するもの (b) serious impairment of smell that is present in the body for a long period of time; 四　次に掲げる音声機能、言語機能又はそしゃく機能の障害 (iv) the following impairments of vocal function, language function, or masticatory function: イ　音声機能、言語機能又はそしゃく機能の喪失 (a) loss of vocal function, language function, or masticatory function; or ロ　音声機能、言語機能又はそしゃく機能の著しい障害であって、長期にわたり身体に存するもの (b) serious impairment of vocal function, language function, or masticatory function that is present in the body for a long period of time; 五　次に掲げる肢体不自由 (v) the following orthopedic impairments: イ　一上肢、一下肢又は体幹の機能の著しい障害であって、長期にわたり身体に存するもの (a) serious impairment of the function of one upper limb, one lower limb, or the trunk that is present in the body for a long period of time; ロ　一上肢又は一下肢のいずれかの指を末節骨の一部以上で欠くもの (b) loss of part of the distal phalange or more of any finger of one upper limb or any toe of one lower limb; ハ　一上肢若しくは一下肢のおや指の機能の著しい障害又はひとさし指を含めて一上肢の三指以上の機能の著しい障害であって、長期にわたり身体に存するもの (c) serious impairment of the function of the thumb of one upper limb or the big toe of one lower limb, or the function of three fingers or more including the index finger of one upper limb that is present in the body for a long period of time; or ニ　イからハまでに掲げるもののほか、その程度がイからハまでに掲げる障害の程度以上であると認められる障害 (d) beyond what is stated in (a) to (c), an impairment of a degree that is found to be equal to or greater than those stated in (a) to (c); or 六　循環器、呼吸器、消化器又は泌尿器の機能の障害であって、長期にわたり身体に存し、かつ、日常生活が著しい制限を受ける程度であると認められるもの (vi) an impairment of the function of cardiovascular organs, respiratory organs, digestive organs, or urinary organs that is present in the body for a long period of time and that is found to be of a degree that significantly restricts the daily life. （報告の期限及び様式） (Time Limit and Form of Report) 第三条　法第三十五条第一項の規定による報告をしようとする者は、その製造又は輸入に係る消費生活用製品について重大製品事故が生じたことを知った日から起算して十日以内に、様式第一による報告書を消費者庁長官に提出しなければならない。 Article 3 (1) Any person intending to make a report under the provisions of Article 35, paragraph (1) of the Act must submit a written report using Form 1 to the Commissioner of the Consumer Affairs Agency within ten days from the day on which the person came to know that serious product incidents have originated with the consumer products that the person manufactured or imported. ２　前項の報告書は、日本語で記載するものとする。 (2) The written report referred to in the preceding paragraph shall be prepared in Japanese. （立入検査の証明書） (Identification Card for an On-Site Inspection) 第四条　法第四十一条第三項の規定により、職員が立入検査をする場合における同条第四項の証明書は、様式第二によるものとする。 Article 4 The identification card referred to in Article 41, paragraph (4) in the case where an official conducts an on-site inspection pursuant to the provisions of paragraph (3) of the same Article shall use Form 2. 附則 Supplementary Provisions この府令は、消費者庁及び消費者委員会設置法（平成二十一年法律第四十八号）の施行の日（平成二十一年九月一日）から施行する。 This Cabinet Office Order comes into effect as of the date on which the Act on the Establishment of the Consumer Affairs Agency and the Consumer Commission (Act No. 48 of 2009) comes into effect (September 1, 2009). 附則（令和元年六月二八日内閣府令第一七号）抄 Supplementary Provisions (Cabinet Office Order No. 17 of June 28, 2019) Extract 第一条　この府令は、不正競争防止法等の一部を改正する法律の施行の日（令和元年七月一日）から施行する。 Article 1 This Cabinet Office Order comes into effect as of the date on which the Act Partially Amending the Unfair Competition Prevention Act, etc. comes into effect (July 1, 2019). 附則（令和七年二月一七日内閣府令第一二号） Supplementary Provisions (Cabinet Office Order No. 12 of February 17, 2025) この府令は、消費生活用製品安全法等の一部を改正する法律の施行の日（令和七年十二月二十五日）から施行する。 This Cabinet Office Order comes into effect as of the date on which the Act Partially Amending the Consumer Product Safety Act, etc. comes into effect (December 25, 2025).
